Don't forget backup and recovery complexity. Stored in the DB, you get that "free". Stored outside, you need to make sure the files are backed up -- and that in a way that is consistent with the metadata stored in the DB.

Yay. It's this sort of crappy bugs that has made me stop working with Oracle and switch to PostgreSQL. Something as basic as this should always work: a regression test suite should pick this up immediately.
