comment How to deny database acess to windows autentication users
Can you remove windows authentication from your SQL server, and just have 1 local account on it? (not 'sa', anything but that)

Right, I'm confusing myself with this scenario: sqlblog.com/blogs/tibor_karaszi/archive/2010/04/16/…

I thought if Witness and Mirror are unavailable, Principal will refuse all transactions?

comment Missing or lost transaction and no errors to show what went wrong
Quick update: We were able to review log using Apex software, however, because we do nightly Full Backup and then afterwards t-log backup, the log was empty. I am investigating ways of preventing this issue. There was a similar t-log truncation question on DBA exchange for anyone interested: dba.stackexchange.com/questions/40666/…

Thank you! (Just a side note: RedGate prepares scripts and it does the version control for me as well--though I am still fine tuning that).

I will have to use ApexSQL tool to read the log, because it is a specific table that has inserts on it daily. I must search the log for a specific ID that would have been on the 2 records that were supposed to be inserted. I don't think something this specific can be found using db_dump_dblog/fn_dblog. I'll update this Question for sure once the log shows if it even contains those 2 records & what happened to them!

I will say though, it's a SQL Server issue not application. I am fairly confident about this.
