C:\Program Files\Microsoft SQL Server\MSSQL11.MSSQLSERVER\MSSQL\DATA\modellog.ldf is the file path. If you cannot recover from a backup, you could just overwrite from another installation of the same version and build number (don't forget to copy the mdf file also). It's unlikely to contain anything of use anyway
